Submit a Claim - Claim Instructions

To qualify for a payment from the Net Settlement Fund, you must submit a timely and valid Claim Form. The Claim Form must be submitted online no later than May 8, 2021 or, if mailed, be postmarked no later than May 8, 2021. Click here to download the Claim Form.
